Form numbering follows the widely used ISEF convention, which many regional and state fairs adopt or adapt. Numbering and requirements change between fairs and between years. Confirm everything against your fair's current published rules.
Nearly every project:
- Checklist for Adult Sponsor — an experienced adult reviews the project and flags which additional forms apply.
- Student Checklist and Research Plan — your written plan: question, procedure, risk assessment, analysis, bibliography.
- Approval Form — signatures from student, parent or guardian, and the review committee.
Added by what your project involves:
- Human participants → informed consent and assent documentation, and committee review before recruitment.
- Vertebrate animals → supervisor qualification and animal-care review.
- Potentially hazardous biological agents → risk assessment and, in most cases, a qualified scientist.
- Hazardous chemicals, activities, or devices → a risk assessment form.
- Work in a regulated research institution → institutional approval documentation.
- Continuation of a previous year's project → a continuation form showing what is new this year.
The sequencing that matters: the sponsor checklist, research plan, and any committee approvals come before experimentation. Everything else can follow. Use the in-app paperwork guide to generate a checklist from your own answers, then take it to your adult sponsor.
